In addition to removing the HTML autoplay attribute from media elements, Disable HTML5 Autoplay also hooks into the media's JavaScript API. This allows for restrictions on media control and simulation of expected behavior to ensure that all pages behave normally. FYI the extension was blocking my telehealth audio and video (which seem to rely on auto-play), as well as playing videos on sites like the weather channel and cnn (the initial advert would play but the main video wouldn't - again because it was using auto-play to move the process along). My telehealth sessions would work in safari but not chrome and it took some debugging to figure this out. Even thought it's not supported anymore, it still worked better than most autoplay blockers. Now it doesn't work on certain sites though, causing my whole computer to freeze when they try to load and play a live streaming video. Generally works on most sites still, but where it doesn't it fails big.
